A) Personal Information and Contact Details

Emergency Medical Information Form
All residents are required to complete this form and submit it as soon as possible after moving in. A copy must be secured to the back of the entrance door to your house/apartment as it contains information vital to Frail Care and or Security staff in the case of an emergency.
Reception can print a copy for you if you do not have access to a printer.

Personal Information Form
All residents are required to complete this form and submit it as soon as possible after moving in.
Reception can print a copy for you if you do not have access to a printer.

Bus Indemnity Form
All residents are required to complete this form and submit it as soon as possible after moving in.  
Reception can print a copy for you if you do not have access to a printer.

Change Vehicle Details Form
Residents who change or re-register their vehicles are required to complete this form and submit it as soon as possible.

Pet Application Form
Approval must be obtained from the Directors before any pet is brought into the Village. There are a number of rules applicable to the keeping of a pet and it is vital that you ensure you can abide by them prior to obtaining a pet.

Garden Landscaping Application
An application for garden landscaping must be accompanied by a detailed plan of the proposed garden including a complete list of all plants and their proposed positioning.

Changes to Unit (Interior & Exterior)
These forms are downloadable PDF documents.
When the form is opened, it can be printed or saved to your device.

These forms cover applications for any changes to or installations on the exterior of your unit, or in the garden. Use them also if you wish to make structural changes within your unit.
You will require additional documents such as supplier quotation and site plan which you can attach to the application and submit to Reception, in the Service Centre

C) GeneralMaintenance & Repair Notification

Use this form to notify the Village Management of a problem you have seen that needs maintenance or repair. It can be something on the Common Property, or it can be within your unit. In the later case, there can be a cost raised for your account for the repair work carried out.
